Payment plans & policy

In an effort to keep dental costs down for you and your family while maintaining a high level of professional care, we have established the following payment plans for our family and cosmetic dentistry services:

  1. Payment is expected at the time of service unless other arrangements have been made in advance.
  2. We accept VISA, MASTERCARD, AMERICAN EXPRESS & DISCOVER credit cards.
  3. We also offer a number of payment plan options that can make more expensive dental and cosmetic procedures affordable to almost any patient. One of these options, CareCredit® provides no interest and extended payment plans. CareCredit can be used for treatment fees of $1,000 or more and allows you to start treatment immediately saving your credit on your credit cards for any unplanned expenses that might arise. With CareCredit, you pay a fixed monthly payment with no annual fees and no prepayment penalties. Our office staff can provide the application or you can sign up online at carecredit.com and receive an immediate decision. CareCredit also offers an automated phone application.

In-House Dental Savings Plan

Effective January 1, 2019

In an effort to encourage good dental health and alleviate rising dental costs, we have developed an “In-House” Dental Savings Plan for our patients who do not have dental insurance or find that our plan costs less than their current dental coverage plan. Since nearly all dental insurance plans require group participation to get any type of price reduction, individuals and families seeking dental insurance on their own often find it too expensive to even consider. Our dental savings plan provides a wide scope of benefits at an affordable price to help minimize the cost of your dental needs. It also offers portability; you are not tied to any one employer. This is your plan and it follows you regardless of who you or your spouse works for during the year.

This is NOT dental insurance. Enrolling in our In-House Dental Savings Plan gives you the opportunity to obtain your dental treatment exclusively at our practice for reduced fees. It cannot be combined with dental insurance or other reduced fee dental plans.

Children's Program: $375

Ages 3-14

  • 15% off all other dental procedures

1 New Adult Patients get 1 Comprehensive and 1 Periodic Exam for a $136 savings.
2 To be eligible for regular cleanings, patients must not have been diagnosed with Periodontal Disease. Patients with a history of Periodontal Disease must enroll in the Periodontal Program.

Benefits & Provisions of the In-House Dental Savings Plan

  • No Annual Deductible
  • No Annual Maximums
  • No Preauthorization’s Required
  • No Waiting Period or Eligibility Periods for Major Treatment
  • No Missing Tooth Clause Restrictions or Exclusions
  • Annual Enrollment Fee for each family member is non-refundable.
  • Annual Enrollment Fees are due at time of enrollment. No exceptions.
  • Enrollment Date begins on the first of the month and when fee is paid in full. Plan expires after 365 days.
  • Family members (who must all have the same address) can enroll at different times of the year. However, the end of the Coverage Year shall remain the same for all members, namely based on the Expiration Date of the first family member to enroll. To maximize coverage, all family members should enroll during the same month.
  • All treatments must be PAID IN FULL at each visit to keep the plan in effect.
  • The exams, bitewing x-rays, cleanings, and fluoride must occur within the year of enrollment and cannot be carried over to the next year.
  • It is the patient’s/parent’s responsibility to make and keep appointments for his/her family members.
  • A fee (based on length of appointment) may incur for each broken appointment without a 48-hour advance notice.
  • Our program is not transferrable to another party or uncovered family member.
  • Participation cannot be combined with any other offer or dental plan.
  • All fees shall be based on our practice’s UCR fees and not any other dental plan’s fees.
  • Annual Enrollment Fees and Procedures are subject to changes during the year.

If you have insurance, we will help you to determine the coverage you have available. We ask that you assign your insurance benefits to us. The balance will be arranged for you to pay as listed above. We will work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.

Please keep in mind that dental insurance is a contract between the patient and the insurance company based to a large degree on the level of premium paid by your employer. Decreased benefits are frequently based on your insurance contract and do not reflect on your truly best treatment options. If you have any problems or questions, please ask our staff.
